Registration

Pre-registration (on or before 1 August 2003)

Registration must be made on the registration form provided. Acceptable forms of payment for pre-registration via MAIL are credit card (MASTER or VISA) or cheque, wire transfer for overseas participants made payable to Saigontourist Travel Service. Registration forms received via FAX are accepted for individuals paying by credit card only (MASTER or VISA). Please note that your signature on your credit card is required in the pre-registration form, as well as a 2-side copy of your credit card and passport with photo. Mail and fax are to be addressed to the Secretariat of IADR/SEA and SEAADE Annual Meeting 2003, at the Faculty of Odonto-Stomatology. You will receive a confirmation of pre-registration once your registration has been processed.

Registration Information

Participants may opt to register with or without accommodation. Full meeting registration includes :

• attendance at all meeting sessions
  
• 3-night accommodation
  
• 6 tea breaks
  
• 3 lunches
  
• Gala dinner
  

If you wish to come earlier or stay behind, hotel accommodation can be extended for you.

Cancellation

Please make your cancellation in writing. Cancellation before 1 August, 2003 will receive a full refund minus 15% processing fee. Refunds will be processed after meeting.

Letter of Invitation

On request, the local organizing committee will be pleased to send an invitation for participant to the meeting. Pleased contact the Secretariat for this invitation if so required. This invitation is only intended to facilitate participants' travel and visa arrangements. It in no way implies the provision of any support, financial or otherwise, from the local organizing committee.

Membership

If you wish to join the IADR/SEA Division in order to enjoy the lower registration fee, your membership must be accepted by IADR Central Office before registration. You may obtain the application form through IARD website at:
